Entrance Requirements
Children may begin attending Grace Lutheran School (GLS) on their second birthday. All children in PK3 and older must be toilet trained.
Students may apply for entrance into preschool after meeting the following requirements:
- Their fourth birthday falls on or before September 1st.
- A valid birth certificate is presented.
- Florida School Entry Health Form (Gold Form DH3040)
- Florida Certificate of Immunization (Blue Form DH 680)
- A parent/guardian and child “Enrollment Interview” with the preschool director has been completed.
Students may apply for entrance into kindergarten after meeting the following requirements:
- Their fifth birthday falls on or before September 1st.
- A valid birth certificate is presented.
- Florida School Entry Health Form (Gold Form DH3040)
- Florida Certificate of Immunization (Blue Form DH 680)
- Complete a parent or guardian and child Enrollment Interview with the school principal.


Admissions Process
All students attending GLS for the first time must meet the requirements listed below:
- Complete an application form plus a $35.00 testing fee (grades 1-8).
- Provide current report card, standardized test, and health records.
- As needed: Teacher informal assessment of child.
- "Shadow day" with prospective class.
- Complete an Admission Interview with principal.
- Pay the registration fee. Please note: A student’s place is not secured until the registration fee is paid in full.
Students entering grade six are required to have a scoliosis screening. Students entering kindergarten and seventh grade are required to submit a physician’s certificate on immunization supplement (Part A-2).
